Who is the service for:
This project is funded by Hertfordshire County Council. It is an intensive outreach support programme for young people who require intensive 1:1 support to improve their housing situation and avoid rough sleeping.
Many of the young people we support have experienced trauma at some point in their lives, which can result in a range of emotional and behavioural challenges. They often lack a stable network of support to help them manage difficult periods in their lives.
"The hyh Intensive Outreach team provided an invaluable service for one of our care leavers who struggles to deal with life’s challenges. An all-round good service. Good communication, Good Team, Great results.
Thank You very much!”
